    “Sophisticated and highly targeted attack”, ie some dumbo click an official looking email and provided their credentials to a spoofed webpage.

      Every. Time.

      The code itself might be highly complex and sophisticated, but it almost always takes a malicious link to get it into the system where it can do its work.
